Therapeutic Use Exemption Form

To provide and support Athletes, their Physicians and Anti-Doping Organisations (ADOs), WADA has provided two checklists, developed by WADA and external medical specialists primarily for the TUE Committee.
The checklists covers most commonly seen conditions, and lists the requirements for completing the TUE application form, including medical evidence that must be submitted along with it.
